Use this page to learn more about requesting a non-urgent appointment with Leading Edge.
This online appointment request form is used to notify Leading Edge that you would like to schedule a non-urgent appointment. Please note that detailed information regarding your exam and appointment with Leading Edge will be discussed on the phone at the time of scheduling. Your request will be processed during normal business hours, Monday-Friday, 8:00 am - 5:30 pm, excluding holidays. Leading Edge scheduling representative will attempt to contact you during the same or next business day. Please provide your primary phone number to contact you during daytime hours. If you provide any additional phone numbers or e-mail address that will make the scheduling faster and easier. Please note we use phone calls, text messaging and e-mails to set up your visits most efficiently.
